This form should be used to request a Mass Intention for a Mass at St. Patrick Church or St. Michael Mission.  

This form may be completed and saved as a PDF document, ready to print or deliver with your payment to the parish office.

This PDF Mass Intention Request form  has fillable fields which may be completed with the computer, then saved and/or printed for delivery with yur payment to the parish office.

This form should be used to request a copy of a certificate for a previously administered Sacrament -- a baptism, First Communion, Confirmation, or Marriage.

This form should be completed and mailed or delivered to the parish office.  Allow 5-10 business days for processing.

Use this form for any event that you are considering in our Parish Hall.  The form is for your information only.  You will need to meet with a representative of SPRCP at least six months prior to your event to work out the details and discuss any questions you may have.
